 Individualized Health Care Plans and 504 Accommodations
 
 Chronic mental and physical health conditions or disabilities can interfere with school participation and achievement. Some students need specialized services and require an Individualized Health Care Plan (IHP).  The need for an IHP is based on required nursing care not educational entitlement.

The school nurse must determine which students require an IHP, prioritizing those students whose healthcare needs affect their daily functioning or safety.  The IHP is developed collaboratively with information from the family, the student, the student's healthcare providers, and school staff, as appropriate (NASAN & ANA, 2005).  The IHP includes medical orders implemented at school. 

The IHP is reviewed at least annually, updated as needed, and revised as significant changes occur in the student's health status or medical treatment.

A 504 plan insures that students with disabilities have the same access to an education as their non-disabled peers.  Identification of students and determination of their eligibility for accommodations under Section 504 must be a collaborative process by a school-based 504 team.  Anyone can make a referral for evaluation of a student's eligibility as long as the school district has reason to believe that the child needs the services under Section 504 because of a disability.

If you believe that your student may qualify for and IHP please complete the Health Care Plan forms and return them to your school nurse.
